Health Assessment for people aged 45 to 49
This health assessment focuses on the needs of people around 45 to 49 years of age who are at risk of developing a chronic disease. The aim of this health assessment is to assist with the detection and prevention of any chronic diseases you may be at risk of developing, to enable early intervention strategies to be put in place to ensure you maintain the best possible long-term health.
Diabetic Risk Assessment for people aged 40 to 49
The Diabetic Risk Assessment is aimed at people aged between 40 and 49 years of age who are at high risk of developing Type 2 Diabetes and utilises the Australian type 2 diabetes assessment tool (AUSDRISK) to assess your risk of developing type 2 diabetes over the next 5 years.
If you have a high result (>12), you are eligible to attend a diabetes risk evaluation with your doctor for further testing and management, and to assess your eligibility for a subsidised Lifestyle Modification Program.
Health Assessments for people aged 75 years and over
If you are aged over 75 (or 55 years and over for Aboriginal and Torres Strait Islanders living in the community) you are eligible for this yearly health assessment. The assessment is conducted by one of our Registered Nurses. You are then seen by your doctor to discuss the outcomes.
The assessment considers all areas of your physical health & wellbeing including diet, exercise, mobility, mental health and relationships. Part of the assessment also involves discussing your living arrangements to ascertain the services available to you for additional help within the home should you need it. It provides a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions in order to improve health and or/quality of life.
