Regular health assessments are an important part of maintaining your long-term wellbeing. At Southern Star Medical, we provide comprehensive preventive health assessments in Southern River to help identify potential health concerns before they become more serious. Our experienced GPs take the time to understand your medical history, family history, lifestyle and individual risk factors to develop a personalised plan that supports your ongoing health.

During your health assessment, we may review your blood pressure, weight, physical activity, nutrition, alcohol consumption, smoking status and other lifestyle factors. We can also discuss recommended health screenings and preventive measures based on your age, medical history and overall health. Our goal is to help you stay healthy through early detection, prevention and proactive healthcare.

Health Assessment for people aged 45 to 49

If you’re aged 45 to 49 and at risk of developing a chronic disease, you may be eligible for a comprehensive health assessment. This assessment focuses on identifying early signs of chronic conditions such as diabetes, heart disease and other chronic illnesses before symptoms develop.

Your GP will review your medical and family history, assess your lifestyle and discuss strategies to reduce your future health risks. Early identification of potential health concerns can help you take appropriate steps to improve your long-term health outcomes.

Where ongoing care is required, your GP may recommend a GP Chronic Condition Management Plan to help monitor and manage an existing or developing chronic condition.

Diabetic Risk Assessment for people aged 40 to 49

If you’re between 40 and 49 years of age and have risk factors for Type 2 diabetes, your GP may recommend a diabetes risk assessment using the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK).

If your assessment indicates an increased risk, your doctor can arrange further investigations, discuss lifestyle modifications and provide ongoing support to help reduce your risk of developing diabetes. Where appropriate, you may also be eligible for additional diabetes prevention and lifestyle management programs.

Maintaining a healthy lifestyle is an important part of reducing your risk of Type 2 diabetes. Our team can provide guidance on nutrition, physical activity and other preventive measures, and may refer you to our Dietitian & Nutrition Services where appropriate.

Health Assessments for people aged 75 years and over

Regular health assessments become increasingly important as we age. Patients aged 75 and over (or 55 years and over for eligible Aboriginal and Torres Strait Islander peoples) may be eligible for an annual health assessment to help maintain their independence, wellbeing and quality of life.

Conducted by one of our Registered Nurses in collaboration with your GP, this assessment reviews your physical health, mobility, nutrition, medications, mental wellbeing, home environment and any support services you may require. The aim is to identify health concerns early, reduce the risk of illness and falls, and help you continue living safely and independently.

Depending on your individual needs, your GP may also recommend other preventive healthcare services, including immunisation and vaccination services and ongoing health monitoring.

Our team provides preventive healthcare for patients of all ages, including women’s health services, men’s health services and children’s health and baby checks.
